University of Utah Graduate School Application Forms and EEGP Application Forms must be completed and submitted following the instructions below.  Once all your information is received and your application fee is paid, the EEGP will review your application. Applications for Fall Semester must be received by January 15. 
 
1. START BY GETTING THE Graduate School Form from:

Graduate School Application Form

1..... THEN SUBMIT the following to the U of Utah Admissions Office (address to the left):
  • UofUtah Application Form
  • Application Fee
  • Official Transcripts
  • Official TOEFL Scores (International Applicants)

 
2.THEN GET THE EEGP Forms from:
